case management

We at Mini Wonders have over 10 years of experience working with case management families and services alike. We have been involved in successful tribunal cases, helped plan and build houses and therapy pools and offer the much needed continuous physiotherapy to so many families.

We currently work with a large number of case management companies and their case managers to ensure the best possible care is carried out for all children, young people and young adults. This includes working closely with local services, attending regular MDT meetings and providing specialist physiotherapy reports.

We recognise the increased demands on families and clients during the initial instruction period, with lots of therapists and professionals involved. We aim to make our assessment process as efficient as possible, gathering as much medical history and physiotherapy findings from other sources as able. We work together with all professionals involved in the client’s care, and where possible, we write joint goals and programmes with other therapists, for a simplified and effective plan. This is something that we have been doing for over a decade with great effect and our longest families have stayed with us the whole way through.

Our extensive initial assessment is carried out either in the community or our clinic, reports and equipment recommendations are all included within the initial assessment report. Our report outlines the client’s requirements, referrals, evidence-based outcome measures, recommendations for physiotherapy intervention, and clinically relevant SMART goals, as discussed and agreed with the client and their family. We provide a list of equipment recommendations, each with its own clinical reasoning form for deputy approval. We appreciate that all suggested mentions within the report must be followed by robust clinical reasoning and evidence.

All of our physiotherapy team are insured for public liability insurance under the Chartered Society of Physiotherapy (CSP) and through our business indemnity insurance. All of the physiotherapy team are also members with the Association of Paediatric Chartered Physiotherapists (APCP) to ensure we are up to date with any changes within our professional world. Our clinical information is held in accordance with the Healthcare Professions Council (HCPC) ethics and standards, General Data Protection Regulation (GDPR) and we also are part of the Information Commissioners Office (ICO). Our physiotherapists and assistant team all have enhanced DBS checks available on request.

All of the above discussed forms are available on request.
